The file is a self extracting "ZIP" file that will extract all the forms into a folder named "BOE_Forms" on your "C" drive. There should be a total of 10 forms in the downloaded file.

The contents of the BOE_Forms.EXE file are:

3. Financial Contract 2001
4. Waiver Document
5. Rights and Responsibilities
6. Consent for Release
7. Emergency Information
8. Medical History
9. Physical Document
10. YMCA Waiver

Each form is in the rich text format (.rtf) and should be easily opened in any word processing program. Make note of the folder where you save the file. Once downloaded, go to that folder, double click on the file name and follow the instructions.
